Renown Health is working with county and state health districts and the Centers for Disease Control and Prevention (CDC) to help distribute the supplies of COVID-19 vaccines to our community. The COVID-19 vaccine has been approved for those 16+. In order to schedule a COVID vaccine for someone aged 16 or 17, please fill out the below form and our Customer Engagement Team will reach out to you within 14 business days to set up an appointment.
Please note: Minors aged 16 or 17 must be accompanied by a parent or guardian at their vaccination appointment.
Anyone 18+ should follow the typical process of scheduling a COVID vaccine appointment through MyChart.
